This is a pilot project for people who are blind or partially sighted and have an additional long term health condition. It is funded for one year by the Derbyshire Clinical Commissioning Groups. The aim is to help people before they need ongoing support from health or social services.
We will work with people to increase the uptake of services already available to them, to maintain or improve their health and social activity.
We will visit you at home, find out about your needs, and together, come up with a plan. We will work with you over six visits to help you reach your goals. This may include accompanying you to identified activities or clubs, while you find your confidence.
We take referrals from family doctors, social workers and other Sight Support Derbyshire services, but you can also contact us yourself.
